Why are we introducing this service

We understand how closely weight, reproductive health, and overall wellbeing are linked. To offer more complete support to our patients, we now provide specialist weight‑loss and fertility consultations with Dr Andrew Robertson. This new service is designed to give you safe, medically guided advice that helps improve general health, support fertility, and prepare your body for a healthy pregnancy.

What is included in the appointment?

A 30‑minute consultation with Dr Robertson is priced at £150. Patients will also be enrolled in a lifestyle support group for continuous support.
During your consultation, you can expect:

  • A comprehensive review of your medical, weight, and fertility history
  • Assessment of medications, lifestyle factors, and any safety concerns
  • A personalised treatment plan tailored to your goals
  • Advice on how to correctly and safely use prescribed medications
  • Fertility and family‑planning guidance, especially if you are preparing for pregnancy
  • A clear explanation of expected benefits, possible side effects, and safety precautions
  • Follow‑up recommendations and a summary letter sent to your NHS GP

Clinical History & Assessment

  • Your past medical history, family history, medication history, and weight history
  • Any previous weight‑loss treatments and your future reproductive plans
  • Pregnancy risk assessment (e.g., last menstrual period)
  • Drug and allergy history
  • Screening for eating‑disorder concerns or red flags
  • Contraindications such as pregnancy, breastfeeding, MEN2/MTC, significant gastrointestinal issues, or a history of pancreatitis

Measurements & Investigations

Where available, the appointment may include:

  • BMI measurement (required)
  • Review of recent blood tests (renal and liver function, HbA1c, thyroid function)
  • Blood pressure and heart rate, using clinic records or patient‑provided readings

Counselling & Treatment Guidance

During this part of the consultation, Dr Robertson will cover:

  • Expected outcomes, safety considerations, and common side effects
  • How to use KwikPen injections, including a demonstration and access to a training video
  • Important safety information on medications such as Mounjaro, Wegovy, and Saxenda, including the rare risk of medullary thyroid carcinoma (MTC) and symptoms to be aware of (neck swelling, swallowing difficulties, breathing difficulties, or persistent hoarseness)
  • Contraception advice, including the requirement for non‑oral contraception during the first month of treatment and for one month after each dose increase. Options may include condoms, oral contraceptives, the Evra patch, or the NuvaRing.
  • How and when to pause treatment if planning to conceive

Documentation & Follow‑Up

At the end of your appointment:

  • Verbal consent is recorded
  • A notification letter is sent to your NHS GP
  • You receive a personalised plan for treatment and next steps

Who is suitable for this program?

Suitable patients

Adults who meet MHRA/NICE criteria, such as:

  • BMI ≥30, or
  • BMI ≥27 with weight-related comorbidities (e.g., PCOS, prediabetes, hypertension, dyslipidaemia).

This is particularly relevant for patients preparing for fertility treatment where weight optimisation is clinically beneficial.

Exclusions

  • Pregnancy or breastfeeding.
  • Planning to conceive sooner than the medication allows for wash-out (1–3 months depending on the drug).
  • Personal or family history of MTC or MEN2.
  • History of pancreatitis.
  • Severe GI disease (e.g., gastroparesis).
  • Hypersensitivity to any component.
  • Patients currently on conflicting therapies.
  • Probable exclusion. unhappy for us to inform GP by letter

Patients must also agree to use non-oral contraception (condoms) during initiation and dose escalation.

What results can patients realistically expect?

Weight loss results depend on the medication used, how consistently it’s taken, and lifestyle changes like diet and exercise. Typical average results come from licensed clinical trials:

In everyday medical care, some differences between patients are normal. The usual goal is to lose at least 5% of body weight within 3 to 6 months, measured while taking the highest dose that is safe and tolerable.

When would you recommend follow-ups?

Follow-up schedule:

  • 4 weeks after initiation
  • At each dose increase (typically every 4 weeks)
  • 8 and 12 weeks
  • Then every 3 months depending on stability and response. 

Follow-ups review:

  • BMI/weight trends
  • Side effects and tolerability
  • Contraception status
  • Planning for conception or fertility treatment
  • Whether continued treatment is clinically beneficial

A summary letter is sent to the patient’s NHS GP after initiation and at key review points.
